[BreakoutBox] Replace MSTP::is_pending_pull_ with a counter |is_pending_pull_| was used to make sure that frames were made available to the stream only if there was a pending pull request. However, if multiple pull requests are issued and then no more pull requests arrive, some of the corresponding read requests will remain unsettled when |is_pending_pull_| becomes false. This CL fixes this by replacing the boolean |is_pending_pull_| with a counter. For historical reasons, there are some small top level directories. Now the guidance is that new top level directories are for product (e.g. Chrome, Android WebView, Ash). Even if these products have multiple executables, the code should be in subdirectories of the product.
