Actually, the rule is that the "city_name" field in the infobox is always just the city's name in isolation. The title of the article can be either "City" or "City, Ontario", depending not on any cross-regional "all titled identically" rule but rather on whether the name is unique or not — for just a few examples, Kenora, Thunder Bay, Timmins and Greater Sudbury are unique names and thus stand alone, while North Bay, Sault Ste. Marie and Espanola aren't unique and thus have to have the "comma Ontario" to distinguish them from the Sault Ste. Marie in Michigan, the Espanola in New Mexico and the North Bay in California.
