Abstract

The present invention relates to a ligament staple, intended to anchor at the surface S of a bone the free end of an artificial tubular ligament L, in particular a joint ligament, which extends in a hole P made in the bone. According to the invention, this staple comprises a plug 3 which can be inserted in the free end of the ligament L and fitted inside the opening 7 through which the latter emerges from the bone. The ligament staple according to the invention more precisely comprises a body 2 which can be pressed into the hole P in the bone and which has a longitudinal bore 7, which is preferably conical, constituting the said opening through which the ligament emerges from the bone, the plug 3 itself having a frustoconical shape which is complimentary to that of this bore 7. In this way a staple/plug is produced which, after implantation, both fastens the bone to the ligament and provides complete leaktightness with respect to leaks of synovia.

Description

Ligament staple, in particular for implanting artificial joint ligaments
The present invention relates to a ligament staple, intended to anchor to the surface of a bone the free end of an artificial tubular ligament, in particular an articular ligament, which extends in a hole made in the bone. .

It is now known to replace damaged internal ligaments of a joint, such as the cruciate ligaments of the knee, with artificial ligaments.

To implant these artificial ligaments, one begins by making, in each of the bones of the joint, a hole which opens, on the one hand, on the condyle and, on the other hand, at a point on the outer surface of the bone located outside the synovial envelope, then the artificial ligament is threaded into these two holes, the free ends of which are then fixed to the bone by conventional staples.

Although this method of implantation of artificial ligaments preserves the integrity of the synovial envelope, synovium leaks can still occur through poorly sealed holes by the ligament and staples. It is therefore not uncommon for implantations of artificial joint ligaments to be followed by effusions of synovia.

The present invention proposes to remedy this drawback and, to do this, it relates to a ligament staple, as described in the preamble, which is characterized in that it comprises a plug insertable into the free end of the ligament and nestable inside the opening through which the latter opens from the bone.

Advantageously, this ligament staple further comprises a body which can be inserted into the bore of the bone and having a longitudinal bore constituting said opening through which the ligament opens out from the bone, the plug having in turn a shape complementary to that of this bore.

There is thus, thanks to the invention of what is agreed to be called a staple-stopper which, after implantation, achieves both the fixation to the ligament bone and a total seal vis-à-vis synovia leaks since, on the one hand, the ligament is firmly wedged between the wall of the body bore and the plug fitted into it and, on the other hand, its open end is completely closed by the latter .

Furthermore, a preferred embodiment of the invention provides that from its opening for insertion of the plug, the bore of the body tapers in a conical shape, over at least part of its length and that the cap has a corresponding frustoconical shape.

This arrangement guarantees permanent attachment of the ligament to the bone, even if the latter is too strongly stressed in traction.

In such a case, in fact, the frustoconical plug would be pulled towards the bottom of the conical part of the bore of the body and would continue to exert its action of blocking the ligament thanks to a wedge effect.

According to another characteristic of the invention, the body of the clip carries, on its insertion end of the stopper, a flattened head projecting laterally, so as to be able to be held in abutment against the external surface of the bone.

In addition, other arrangements, such as the presence of a rounded shoulder at the end of the conical part of the bore of the body and / or the formation of radial grooves on the wall thereof, participate in maintaining place the ligament in the staple.

A preferred embodiment of the present invention will now be described in more detail, but only by way of non-limiting example, with reference to the accompanying drawing in which
FIG. 1 is a schematic view in partial section of the knee joint of a patient, in which an artificial ligament has been implanted using staples in accordance with this embodiment of the invention and
FIG. 2 is an enlarged sectional view of a ligament staple visible in FIG. 1.

In the knee joint shown in Figure 1, the crossed natural ligaments, which were damaged, were replaced by artificial ligaments, of which only one L was drawn. In a manner known per se, these artificial ligaments are tubular and are made of a synthetic material which is generally "Dacron".

To implant each of these artificial ligaments L, one begins by practicing two holes P, respectively in the femur F and the tibia T, by making them lead on the one hand on the external surface S of these bones, at points located outside of the synovial envelope E, and on the other hand on the condyles Cen two points distant from each other. Then, we thread the artificial ligament L into the two holes P, between which it thus crosses the articular gap A, and finally we fix the free ends of the ligament on the outer surfaces S of the bones, using staples 1, la, one of which 1 is shown on a larger scale in FIG. 2.

As can be seen, this clip, which in fact constitutes the subject of the present invention, consists of two distinct elements, namely a tubular body 2 and a plug 3 which can be fitted into the latter, these two elements preferably being produced stainless steel.

The body is more precisely formed by a short rod 4 which carries, at one of its ends, a flattened head 5 projecting laterally and which tapers very slightly towards its free end 6 opposite the head 5.

The body 2 also has a longitudinal central bore 7 open at its two ends. From its opening 8 opening onto the head 5, this bore 7 tapers in a substantially conical shape over part of its length and, beyond a rounded shoulder 9, it ends in a mouth 10 which widens towards the free end 6 of the rod 4. It will also be noted that near the shoulder 9, the wall of the conical part 11 of the bore 7 has radial grooves 12.

The plug 3 is in turn constituted by an elongated frustoconical part, having substantially the same conicity as the part 11 of the bore 7 while having transverse dimensions slightly smaller than those of the latter.

To fix an artificial ligament using the clip that has just been described, the procedure is as follows.

First, we slightly extract the free end of the ligament
L out of the hole P and it is threaded into the bore 7 of the body 2 of the clip, by; 'introducing through the mouth 10 of the latter, whose flared shape facilitates this operation. Then, while retaining the ligament, the body 2 is embedded in the bore P of the bone, previously machined to the shape of the rod 4, until its head 5 is in contact with the surface S of the bone. Finally, after having cut the part of the ligament emerging from the head 5, the plug 3 is forced into place, by its tapered end, in the conical part 11 of the bore 7 of the body 2. It follows that the end free ligament L is wedged between body 2 and the plug 3 which also provides a seal against synovia leaks.

To perfect this seal, you can optionally heat the
ligament L around the plug 3 so that the synthetic material of which it is made, adhere by softening to the plug and to the head 5 of the body 2.

It should also be noted that the shoulder 9 and the grooves 12 in the bore of the body 2 oppose any sliding of the ligament L towards the inside of the bone. Despite everything, even supposing that such a shift takes place, in the
for example, if the ligament is under too high a tensile stress, the plug 3 would be slightly pulled towards the bottom of the conical part 1 of the bore
7 and would continue to exert its blocking action a little more deeply on
the ligament.

In the foregoing, a stopper clip has been described, the head 5 of which is substantially perpendicular to the longitudinal axis of the bore 7 of the body 2.

However, it goes without saying that the invention also covers staples-stoppers whose head will be inclined relative to this axis to allow the anchoring of an artificial ligament on a bone surface not perpendicular to the axis of the drilling from which it emerges. Such an inclined head staple is shown in la in FIG. 1.
