Abstract

Ce cotyle 2 possède tout d'abord une surface intérieure 21r définissant un volume intérieur V sensiblement hémisphérique. Ce dernier présente une forme de disque, au centre duquel est ménagé un fût taraudé 116, destiné à coopérer avec l'extrémité filetée de la tige 106. Ce corps 114 est en outre creusé d'une échancrure 118, présentant à peu près une forme de triangle, qui est destinée à former une surface de butée pour un tiroir, comme on le verra dans ce qui suit. Au voisinage du fût 116, le corps 114 est creusé de trois lumières traversantes 120 alors que, à sa périphérie extérieure, il est creusé de trois entailles 122. Au voisinage de chacune de ces entailles, un plot correspondant 124 s'étend axialement, à l'opposé du manche 4. Chaque plot 124 présente une dimension transversale, ou largeur, qui lui permet d'être introduit dans une encoche 10 correspondante. Par ailleurs, chaque plot 124 possède une face oblique 1241r de sorte qu'il est propre à prendre appui contre la face oblique 1021de l'encoche 10. L'ancillaire 102 se compose en outre d'un tiroir 126, illustré plus particulièrement sur la figure 6. Ce dernier, qui est de forme globalement triangulaire, est propre à venir en butée contre les parois de l'échancrure 118. Ce tiroir est creusé d'un orifice central 128, à partir des parois duquel s'étendent trois pattes axiales 130, dont les surfaces intérieures définissent des portions de taraudage. Le tiroir 126 est en outre pourvu de dents 132, propres à pénétrer axialement dans les entailles 122 ménagées dans le corps 114. Chaque dent 132, qui s'étend à partir d'une embase 134, possède en outre un épaulement 1321 qui définit une pointe terminale 1322. A l'opposé des pattes 130, le tiroir 126 est muni de différentes cornières, formant des crochets 136. L'un de 7 ces crochets est associé à un pion 138, contre lequel les doigts 108" du fourreau 108 sont propres à venir en butée. Enfin, l'ancillaire de pose 102 comporte un dôme 140, de forme globalement hémisphérique, qui est propre à être reçu dans le volume intérieur V du cotyle 2. Ce dôme 140 définit un fût central 142, dont les parois extérieures sont filetées de façon à coopérer avec les portions de taraudage des pattes 130. Ce fût 142 est en outre creusé d'un logement, qui définit un siège 144 pour un ressort 146, dont l'autre extrémité est propre à venir en appui contre le corps 114, au voisinage des parois bordant le fût central 116. En vue du montage de l'ancillaire 102, il s'agit tout d'abord d'introduire les pattes 130 dans les lumières 120, puis de disposer le ressort 146 entre les parois en regard du dôme 140 et du corps 114. On comprime alors ce ressort, de manière à visser le fût 142 dans les pattes 130. Par ailleurs, on dispose le fourreau 108 autour de la tige 106, que l'on visse dans le fût taraudé 116. En l'absence de contrainte extérieure, le ressort de rappel 146 repousse le dôme 140 et, par conséquent, le tiroir 126 à l'opposé du manche 104. Ce tiroir 126 prend donc appui contre les parois de l'échancrure 118, qui forment ainsi des surfaces de butée. De plus, les dents 132 se trouvent dans une position avancée, en ce sens qu'elles s'étendent dans les entailles 122 et que leurs pointes 1322 sont disposées au voisinage des plots 124. Si l'on désire solidariser le cotyle 2 à l'ancillaire de pose 102, on bascule tout d'abord la bague 110 à l'opposé de la tige 106, de manière à pouvoir disposer d'un débattement maximal en translation du fourreau. Puis, on repousse ce dernier le long de la tige, au voisinage du corps 114, et on fait tourner ce fourreau de façon que ses doigts 108" viennent en prise avec les crochets 136. On 8 notera que ce mouvement en rotation est limité par le pion de butée 138, dont est muni l'un de ces crochets 136, qui permet en outre une fixation par encliquetage élastique au niveau d'un renfoncement 109 d'un doigt 108" correspondant (voir figure 7C). Le chirurgien exerce alors une traction sur le fourreau 108, selon la flèche F, par l'intermédiaire des poignées 108', de sorte qu'il déplace axialement le tiroir 126 à l'encontre du ressort 146 (voir figures 7A et 7B). Ce déplacement du tiroir 126 induit également la mise en mouvement des dents 132, qui reculent vers une position escamotée, en ce sens qu'elles ne sont plus disposées au voisinage des plots 124. Dans ces conditions, de façon analogue à ce qui est décrit dans EP-A-O 190 982, il est possible d'introduire chaque plot 124 dans une encoche correspondante 10. De la sorte, les faces obliques respectives 1021 et 1241 de ces encoches et de ces plots se trouvent en appui mutuel. On notera que, dans cette position, les plots n'occupent pas l'intégralité de l'encoche, de sorte qu'ils définissent un espace libre. Une fois les plots reçus dans les encoches, le chirurgien relâche l'action qu'il exerçait sur le fourreau 108. Dans ces conditions, le ressort 146 rappelle le tiroir 126 en direction du corps 114, de sorte que les dents 132 retrouvent alors leur position avancée initiale. Dans ces conditions, la pointe 1322 de ces dents pénètre dans les encoches 10 (voir figure 8), de manière à combler l'espace libre précité, ménagé au préalable entre les faces droites en regard des encoches 10 et des plots 124. L'ancillaire de pose se trouve alors solidarisé de façon fiable au cotyle 2, puisque la partie évasée des plots 124 est bloquée en position par les parois en regard des encoches 10. Le chirurgien bascule alors la bague 110 en direction de la tige 106, de manière à éviter tout déplacement axial intempestif du fourreau 108. Le chirurgien est ensuite à même d'implanter le cotyle, grâce à une technique classique d'impaction, dans le corps du patient. Puis, il déplace à nouveau axialement le fourreau 108, de manière à retirer les dents 132 des encoches 10, afin de désolidariser l'ancillaire 102 par rapport au cotyle, une fois ce dernier implanté. L'invention permet d'atteindre les objectifs précédemment mentionnés. En effet, l'ancillaire conforme à l'invention autorise une pose particulièrement efficace du cotyle prothétique, dans la mesure où la solidarisation entre cet ancillaire et ce cotyle est d'une grande fiabilité. Ainsi, on notera que, en l'absence de contrainte extérieure, le ressort 146 rappelle en permanence les dents 132 dans leur position avancée, de sorte que les plots 124 ne peuvent se dégager des encoches 10. Dans ces conditions, toute désolidarisation intempestive du cotyle par rapport à l'ancillaire se trouve évitée. En outre, le cotyle prothétique de l'invention présente une surface intérieure entièrement lisse, qui lui permet de recevoir directement une tête métallique de prothèse. En d'autres termes, l'invention permet de s'affranchir de l'utilisation d'un insert, intercalé entre ce cotyle et cette tête de prothèse. A cet égard, on notera que l'existence de voiles pleins, séparant les encoches par rapport au volume intérieur du cotyle, est avantageuse. En effet, ces voiles permettent de conférer un caractère lisse à cette surface d'intérieure, ainsi qu'un caractère continu au débouché de cette même surface intérieure. The slide 126 is further provided with teeth 132, able to penetrate axially into the notches 122 in the body 114. This spring is then compressed, so as to screw the drum 142 in the tabs 130. Furthermore, the sleeve 108 is arranged around the rod 106, which is screwed into the threaded barrel 116. In the absence of external stress, the return spring 146 pushes the dome 140 and, consequently, the slide 126 opposite the handle 104. This slide 126 thus bears against the walls of the notch 118, which form as well as abutment surfaces. In addition, the teeth 132 are in an advanced position, in that they extend into the notches 122 and their points 1322 are arranged in the vicinity of the studs 124. If it is desired to secure the acetabulum 2 to the ancillary laying 102, it first tilts the ring 110 opposite the rod 106, so as to have a maximum displacement in translation of the sleeve. Then, the latter is pushed back along the rod, in the vicinity of the body 114, and this sleeve is rotated so that its fingers 108 "engage with the hooks 136. It will be noted that this movement in rotation is limited by the abutment pin 138, which is provided with one of these hooks 136, which further allows an elastic latching attachment at a recess 109 of a corresponding finger 108 "(see Figure 7C). The surgeon then exerts traction on the sheath 108, according to the arrow F, through the handles 108 ', so that it moves the slide 126 axially against the spring 146 (see Figures 7A and 7B). This displacement of the slide 126 also induces the setting in motion of the teeth 132, which retreat towards a retracted position, in that they are no longer disposed in the vicinity of the studs 124. Under these conditions, in a manner similar to that described in EP-A-0 190 982, it is possible to introduce each stud 124 into a corresponding notch 10. In this way, the respective oblique faces 1021 and 1241 of these notches and these pads are in mutual support. Note that in this position, the pads do not occupy the entire slot, so they define a free space. Once the studs received in the notches, the surgeon releases the action he exerted on the sleeve 108. In these conditions, the spring 146 recalls the drawer 126 towards the body 114, so that the teeth 132 then find their initial advanced position. Under these conditions, the tip 1322 of these teeth penetrates into the notches 10 (see FIG. 8), so as to fill the aforementioned free space, made beforehand between the straight faces opposite the notches 10 and the studs 124. The ancillary installation is then secured reliably to the acetabulum 2, since the flared portion of the studs 124 is locked in position by the walls opposite the notches 10. The surgeon then switches the ring 110 towards the rod 106, so to prevent inadvertent axial displacement of the sleeve 108. The surgeon is then able to implant the acetabulum, using a conventional impaction technique, in the body of the patient. Then, it moves axially again the sleeve 108, so as to remove the teeth 132 of the notches 10, in order to separate the ancillary 102 relative to the acetabulum, once the latter implanted. The invention achieves the previously mentioned objectives. Indeed, the ancillary according to the invention allows a particularly effective installation of the prosthetic acetabulum, insofar as the connection between the ancillary and the acetabulum is of great reliability. Thus, it will be noted that, in the absence of external stress, the spring 146 permanently recalls the teeth 132 in their advanced position, so that the studs 124 can not disengage from the notches 10. Under these conditions, any inadvertent disconnection of the acetabulum compared to the ancillary is avoided. In addition, the prosthetic acetabulum of the invention has a fully smooth inner surface, which allows it to directly receive a metal prosthesis head. In other words, the invention makes it possible to dispense with the use of an insert, interposed between this acetabulum and this prosthesis head. In this respect, it will be noted that the existence of solid webs, separating the notches relative to the internal volume of the acetabulum, is advantageous. Indeed, these sails allow to confer a smooth character to this interior surface, and a continuous character at the outlet of the same interior surface. This makes it possible to reduce the wear of the acetabulum and to avoid the formation of metal particles that can migrate into the patient's body and be toxic to the patient.

REVENDICATIONS 1. Ancillaire (102) de pose, pour un cotyle prothétique (2) qui possède plusieurs encoches (10) définissant un col intermédiaire (101) ainsi qu'une région d'extrémité (102) de plus grande dimension transversale, cet ancillaire comportant un manche de préhension (104), ainsi qu'un embout (112) qui possède un corps (114) pourvu de plots (124) propres à pénétrer dans les encoches (10) du cotyle (2), de façon à définir différents espaces libres, cet embout comportant également un tiroir (126) pourvu de dents (132) mobiles entre une position avancée, dans laquelle elles occupent au moins partiellement lesdits espaces libres, de façon à solidariser cet ancillaire (102) par rapport au cotyle (2), ainsi qu'une position rétractée, cet ancillaire comportant en outre un organe (108) de déplacement du tiroir (126) vers sa position rétractée, actionnable par un utilisateur, ainsi que des moyens (146) de rappel du tiroir (126) vers sa position avancée.  1.
