When we have an SHPC-capable bridge with a second SHPC-capable bridge
below it, pushing the upstream bridge's attention button causes a
deadlock.

The deadlock happens because we use the shpchp_wq workqueue to run
shpchp_pushbutton_thread(), which uses shpchp_disable_slot() to remove
devices below the upstream bridge. When we remove the downstream bridge,
we call shpc_remove(), the shpchp driver's .remove() method. That calls
flush_workqueue(shpchp_wq), which deadlocks because the
shpchp_pushbutton_thread() work item is still running.

This patch avoids the deadlock by creating a workqueue for every slot
and removing the single shared workqueue.

Here's the call path that leads to the deadlock:

shpchp_queue_pushbutton_work
queue_work(shpchp_wq) # shpchp_pushbutton_thread
...

shpchp_pushbutton_thread
shpchp_disable_slot
remove_board
shpchp_unconfigure_device
pci_stop_and_remove_bus_device
...
shpc_remove # shpchp driver .remove method
hpc_release_ctlr
cleanup_slots
flush_workqueue(shpchp_wq)

This change is based on code inspection, since we don't have hardware
with this topology.

Use non-ordered workqueue for attention button events.

Attention button events on each slot can be handled asynchronously. So
we should use non-ordered workqueue. This patch also removes ordered
workqueue in shpchp as a result.

486b10b9f4 ("PCI: pciehp: Handle push button event asynchronously") made
the same change to pciehp. I split this out from a patch by Yijing Wang

Current SHPCHP driver uses msleep_interruptible() function to wait for
a command completion event. But I think this would cause an unnecessary
long wait until timeout, if command completion interrupt came before
task state was changed to TASK_INTERRUPTIBLE. This patch fixes this
issue. With this patch, command completion becomes faster as follows:

o Without this patch

# time echo 1 > power

real 0m4.708s
user 0m0.000s
sys 0m0.524s

o With this patch

# time echo 1 > power

real 0m2.221s
user 0m0.000s
sys 0m0.532s

I recently picked up my older work to remove unnecessary #includes of
sched.h, starting from a patch by Dave Jones to not include sched.h
from module.h. This reduces the number of indirect includes of sched.h
by ~300. Another ~400 pointless direct includes can be removed after
this disentangling (patch to follow later).
However, quite a few indirect includes need to be fixed up for this.

In order to feed the patches through -mm with as little disturbance as
possible, I've split out the fixes I accumulated up to now (complete for
i386 and x86_64, more archs to follow later) and post them before the real
patch. This way this large part of the patch is kept simple with only
adding #includes, and all hunks are independent of each other. So if any
hunk rejects or gets in the way of other patches, just drop it. My scripts
will pick it up again in the next round.

that can be inferred through other debug prints.

State information is currently stored in per-slot as well as
per-pci-function data structures in shpchp. There's a lot of
overlap in the information kept, and some of it is never used.
This patch consolidates the state information to per-slot and
eliminates unused data structures. The biggest change is to
eliminate the pci_func structure and the code around managing
its lists.

Reduce the SHPC hotplug driver's dependence on ACPI. We don't
walk the acpi namespace anymore to build a list of bridges and
devices. The remaining interaction with ACPI is to run the
_OSHP method to transition control of hotplug hardware from
system BIOS to the shpc hotplug driver, and to run the _HPP
method to get hotplug device parameters like cache line size,
latency timer and SERR/PERR enable from BIOS.

Note that one of the side effects of this patch is that shpchp
does not enable the hot-added device or its DMA bus mastering
automatically now. It expects the device driver to do that.
This may break some drivers and we will have to fix them as
they are reported.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
